Role Overview
Our client is seeking a Remote Patient Care Coordinator (Cardiology Patient Navigator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. The ideal candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume caseload of 150+ patients across multiple priorities.
Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, telehealth, utilization review, or home health will be competitive. What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan.

Ensure all patient interactions and records meet HIPAA compliance standards
Support billing documentation and insurance coordination as needed
